None of the implementers of the submit_bio_hook use the bio_offset
parameter, simply remove it. No functional changes.

Here you could also add the removal of bio_offset from async_submit_bio.

Actually no, we can't since for DIO case we do use it from:
btrfs_submit_dio_bio->btrfs_wq_submit_bio
