Insert documentation for structs, enums and functions at header file.
Format existing and new comments at struct blk_mq_ops as
kernel-doc comments.

Hello,

This patch is an effort to enhance the documentation of the multiqueue
API. To check if the comments are confirming to kernel-doc format, you
first need to apply a patch[1] solving a bug around the `__cacheline...`
directive. Then, just run

./scripts/kernel-doc -none include/linux/blk-mq.h

to check that there is no warning at this documentation. I've done my
best at the source to check the purpose of each struct and its members,
but please let me know if I get something wrong.

It makes sense, but it looks like PDU stands for protocol unit request.
Could we have it "PDU(Protocol Data Unit)" ?

Hi André,

Seeing the documentation being improved is great. However, this patch 
conflicts with a patch series in my tree and that I plan to post soon. 
So I would appreciate it if this patch would be withheld until after my 
patch series has been accepted.

I like improving how much is documented, but I absolutely don't like how
everything is pulled into one section above the struct instead of being
documented inline instead.

I realize this probably makes it easier to make nice external
documentation, but imho that's absolutely secondary to having the
documentation being right there where you read the code.

Could you tell me the tree/branch that you had applied Bart's "[PATCH
0/8] Block layer patches for kernel v5.5"? I'll need to rebase my work
on top of that because of the patch "[PATCH 6/8] block: Document all
members of blk_mq_tag_set and bkl_mq_queue_map".

Hi Jens,

If you prefer this style you may want to update
Documentation/doc-guide/kernel-doc.rst. I think that document recommends 
another style for documenting struct members, maybe because that style 
requires less vertical space:

You can replace the ``struct`` in the above example with ``union`` or
``enum``  to describe unions or enums. ``member`` is used to mean struct
and union member names as well as enumerations in an enum.

lines, and ends with a member description, a blank comment line, or the
end of the comment block.

The same documentation also suggests that one can use inline comments:

In-line member documentation comments

The structure members may also be documented in-line within the definition.
There are two styles, single-line comments where both the opening
``/**`` and
closing ``*/`` are on the same line, and multi-line comments where they
are each
on a line of their own, like all other kernel-doc comments::
