[v7,2/5] ceph: add caps perf metric for each session

This will fulfill the cap hit/mis metric stuff per-superblock,
it will count the hit/mis counters based each inode, and if one
inode's 'issued & ~revoking == mask' will mean a hit, or a miss.

Summary:

I think counting this stuff is useful, but I'm not sure you're doing it
in the right places below. Also, you're calling __ceph_caps_metric from
many places where you already know whether it's a hit or miss. You could
just bump the counter and do less work in those cases.

Many of the callers of this function already know whether they are
dealing with a hit or miss. I think walking the rbtree and doing all of
this extra work is not the right approach for those places. You should
aim to bump the appropriate counters at the points where the code makes
that determination.

These checks are somewhat racy. The state could change between where the
spinlock is dropped and where you do the operation. I think you may need
to plumb this deeper down into the places where the decision about
whether to request caps from the MDS is made.

We know this is a hit. We don't need to call this to walk the rbtree
again. I think this should just be a bump of the hit counter.

Hmm. This counts hits/misses even when the caps don't matter  -- i.e. in
the case where the strncmp returns 0, or the mount option check fails.
It seems like this should not be used here, and you should just count
the hits and misses inside the if/else blocks.

This spot of the code, we're doing an open and are pre-requesting caps.
We're not actually planning to use them at this point. We're issuing a
call to the MDS either way. Should we even count this at all?

There is no guarantee that you'll still have all of CEPH_STAT_CAP_INODE
once you go to issue the getattr below. It seems like this might be
better counted in __ceph_do_getattr itself.

This call has the "force" flag set to true, so it turns out the cap mask
