Add support for the Adreno 619 GPU, as found in Snapdragon 690 (SM6350),
480 (SM4350) and 750G (SM7225).

I think we typically provide just a single cmd entry. You also have 
ddr_cmds_num = 1, so the rest of the entries are unused and can be removed.
