This simplifies error handling paths and generallly removes a bunch
of boilerplate.

Since RFC:
 - Use unreachable() rather than return -EINVAL; to convince compiler
   that the code is really unreachable and stop it complaining.
 - Reduce scope of int ret in resume() callback.
