NFS: move pnfs layouts to nfs_server structure

Layouts should be tracked per FSID (aka superblock, aka struct nfs_server)
instead of per struct nfs_client, which may have multiple FSIDs associated
with it.

I actually thought of suggesting that yesterday but
I'm not sure fsid is unique per nfs_server.  Is it?

Ah!  I incorrectly assumed that fsids are unique.  client.c:nfs_clone_server() proves otherwise.
