[v3] NFSv4: Fix reboot recovery in copy offload

Copy offload code needs to be hooked into the code for handling
NFS4ERR_BAD_STATEID by ensuring that we set the "stateid" field
in struct nfs4_exception.

v3 change: aside from the pointer issue in _nfs42_proc_copy() that was
noted by Olga, I went back to the original design for recovery. The
fact that this is NFSv4.2 means that we should be calling TEST_STATEID
before determining that the BAD_STATEID applies.
