[v4,28/78] ncr5380: Drop DEF_SCSI_QCMD macro

Remove the DEF_SCSI_QCMD macro (already removed from atari_NCR5380.c). The
lock provided by DEF_SCSI_QCMD is only needed for queue data structures.
