[4/9] PCI: pciehp: Unify controller and slot structs

pciehp was originally introduced together with shpchp in a single
commit, c16b4b14d980 ("PCI Hotplug: Add SHPC and PCI Express hot-plug
drivers"):

shpchp supports up to 31 slots per controller, hence uses separate slot
and controller structs.  pciehp has a 1:1 relationship between slot and
controller and therefore never required this separation.  Nevertheless,
because much of the code had been copy-pasted between the two drivers,
pciehp likewise uses separate structs to this very day.

The artificial separation of data structures adds unnecessary complexity
and bloat to pciehp and requires constantly chasing pointers at runtime.

Simplify the driver by merging struct slot into struct controller.
Merge the slot constructor pcie_init_slot() and the destructor
pcie_cleanup_slot() into the controller counterparts.  Rename the slot
struct's "lock" member to "state_lock" and rename "work" to "button_work"
for clarity.

No functional change intended.

Hm, you mean split out the renaming of the "lock" and "work" members?

The unification of the two structs needs to happen in a single commit
to avoid breaking the build for people doing a git bisect.

Could the rename be done in a separate patch before the unification?

Okay, I've already reworked my internal development branch to do the
unification first, then the rename, but can easily turn the order around,
no problem at all.

The preceding patches 1 to 3 of this series could be merged independently,
or would you prefer me to resend them?  The first one is also included in
Keith's error handling rework series (posted yesterday):

Important side note:  Keith's series includes a pciehp fix which should
probably go into 4.19:

Now, if that patch goes into, say, 4.19-rc3, the pci/hotplug branch would
have to be rebased on top of 4.19-rc3 to avoid conflicts during the 4.20
merge window.

The problem with unification of the controller and slot structs in pciehp
is that it's a fairly intrusive patch.  Any pciehp patches that are pending
now would need to be rebased on top of it.  Right now the only pending
pciehp patches are Keith's series posted yesterday and this patch from me
(which would be 4.20 material):

I can rebase all the patches as often as necessary, no problem.
Just let me know when it is most convenient for you to merge what

I don't care which order they're in.  I just wasn't sure if the
unification was possible without the renaming.

I applied patches 1-3 to pci/hotplug.  If you could rebase what's left
on top of that, it would be great.

OK, I rebased pci/hotplug to v4.19-rc4.  Any chance you could update
this series to apply on top of that?

Only a single patch of that series is affected by the rebase, patch [2/8].
I have just marked that patch as "Not Applicable" in patchwork and am
including a replacement patch below.  Same patch, just rebased on top of
current pci/hotplug branch.

I can also post the series again if that is easier for you to handle
than a replacement patch.
